Utah Medical Cannabis Card

Individuals with one or more qualifying conditions are authorized under the Utah Medical Cannabis Act to receive a medical cannabis patient card to treat specific qualifying conditions. I am a licensed QMP in the state of Utah.
The qualifying conditions that are within my scope of practice to treat are Posttraumatic Stress Disorder (PTSD) and Autism Spectrum Disorders (ASD).  Treatment entails an initial evaluation, signing a release of information to the Utah Dept. of Health to allow me to inform them of condition being treated, relevant symptoms and treatment details, and a 6 month follow up, then follow up every 6-12 months on and on going basis or whenever changes to treatment need to be made.

Insurance companies will not be billed for Medical Cannabis Services.

 

RESTRICTIONS- Cannabis cards may not be issued to Federal Employees or CDL drivers. I advise you to consult a Utah attorney first.

Steps to Obtain a Medical Cannabis Card in Utah

1. Locate a Qualified Medical Provider (QMP) Find a medical provider registered to recommend medical cannabis. You can access a list of registered QMPs through the Utah Department of Health (UDOH) website. You must be 21 years old in the state of Utah to qualify for a medical cannabis card if you are under 21 and have a more critical need, the Compassionate Use Board may be petitioned. This requires signing guardianship to someone and setting a meeting with your guardian to discuss your case. 2. Meet with Your QMP Schedule a visit with your QMP. A copy of your driver's license is required. Upload your state ID Provide provider with the last 4 of SSB. During this visit, they will assess your condition and medical history and document the evaluation. 3. Apply for Your Medical Cannabis Card Go online to Utah's Electronic Verification System (EVS) to begin your medical cannabis card application: https://medicalcannabis.utah.gov/patients. Use the legal name on your ID to register and ensure that name is the same name used to register the PHR account. Let your provider know when this step is complete. 4. QMP Recommendation After starting your online application, your QMP must complete their portion of the form, which includes their recommendation for medical cannabis treatment. Steps to Obtain a Medical Cannabis Card in Utah 5. Pay the Application Fee Log back into the EVS and pay the $15 application fee online. Your application will then be sent to UDOH for review. 6. Receive Your Medical Cannabis Card If approved, you'll receive your card via email or you can print it from the EVS site. 7. Schedule Initial visit at the Pharmacy with a licensed Pharmacist to review best options. 8. Renewal URenewal is required every 6-12 months, depending on various factors. An appointment is required to renew your card.
